Developing a Guided Online Self-Assessment

Project aims

Working with Adam (now part of Access group) to increase the senior leadership teams awareness of models of self-directed support and personal budgets. To work with Adam developers to create a guided self assessment linked back to the Care Act 2014 eligibility and the National Continuing Healthcare Eligibility criteria.

Project description and outcomes

Self Directed Futures was commissioned to deliver workshops and to provide advice and support to the senior leadership team around self directed support, Individual Service Funds, micro providers, introductory agencies and options for managing the money. A guided self assessment was developed for use as part of an online support offer that Adam was developing for self-funders and people looking to find care and support.
