Individual Service Funds Evaluation

Project aims

Working with Warwickshire County Council commissioning develop a methodology for evaluating their Individual Service Fund domiciliary care and support framework over 18 months.

Project description and outcomes

Self Directed Futures worked alongside Warwickshire County Council commissioning team and Individual Service Funds programme lead to develop a questionnaire and set of metrics to show outcomes and progression for people taking an Individual Service Fund.
