Third Party Personal Health Budgets

Project aims

To support Wiltshire Integrated Care Board and Lives Through Friends with market shaping, and the creative use of personal budgets to discharge three people with learning disabilities and/or autism from local NHS Assessment and Treatment units that they had been stuck in for several years (due to lack of local specialist support provision and flexible budgets).

Project description and outcomes

Self Directed Futures was commissioned to work alongside Lives Through Friends to provide advice, guidance and tactical commissioning support to enable the creative use of Individual Service Funds (Third Party Personal Health budgets) to expedite people's discharge. 

We worked along Lives Through Friends to develop costed support plans that enabled good quality support teams to be assembled, and all three individuals were discharged within 9 months into their own homes with bespoke support.
